WHAT IS THE ALACHUA COUNTY SPECIAL NEEDS PROGRAM?

The Special Needs Program is a registry of individuals in Alachua County who have special medical needs and may need emergency sheltering and/or evacuation assistance in the event of a disaster.  Being a part of this program lets Alachua County Emergency Management and emergency response personnel know that you may need help before, during or after a disaster.  Registration is FREE.

 

WHO IS ELIGIBLE? 

The Special Needs Program is designed to provide people with transportation to shelters during a disaster evacuation due to a hurricane, flood or wildfire. The program registration is free, voluntary and confidential. Registration is based on persons meeting the Special Needs Program's criteria. Registrants are responsible for keeping their registration information accurate and up to date and renewing their registration each year.

Alachua County Emergency Management maintains the Special Needs registry of disabled persons within its jurisdiction in accordance with the Florida Statutes Chapter 252.355 - Registry of Persons with Special Needs.

 

WHO ARE PEOPLE WITH SPECIAL NEEDS?

 People who have certain physical or mental conditions or limitations, and need daily assistance by a caregiver or health provider for personal care (bathing, eating, walking, transportation, etc.) and/or medication administration or treatment for stable conditions such as:

 

WHO IS ELIGIBLE?

Any residents of Alachua County with Special Needs who:

  

WHO IS NOT ELIGIBLE?

Examples: Alzheimers (advanced), Bedbound (permanent), Cardiac (unstable), Comatose, Contagious Condition (severe), Cystic Fibrosis (late stage), Psychosis (uncontrolled), Seizures (uncontrolled), Terminal Illness (final stage)

 

 

Examples: Arthritis/Osteoporosis (able to walk), Asthma, Bronchitis, Common Cold or Flu, Diabetics (self-managed), Edema, Emphysema (non-oxygen-dependent), Bone Fracture, High Blood Pressure, Hypoglycemia, Knee Replacement, Medication Maintenance, Migraine Headaches, Sleep Apnea

This registry is CONFIDENTIAL. The listing is maintained by Alachua County Emergency Management and shared only with the County Public Health Unit and first responders to meet the needs of the people enrolled in the program.

 

WHAT IF MY INFORMATION CHANGES AFTER I HAVE ALREADY REGISTERED? 

Keep your information current!

You are responsible for informing Alachua County Emergency Management of any changes that may occur and affect your registration records. For example, if you move, change your telephone number, or no longer need to be registered, let Alachua County Emergency Management know immediately so your information can be updated. If you cannot be contacted during an emergency evacuation, you cannot be helped.
